Sorry if this comes off as confusing.
I have written a script using the NodeJS request module that runs and performs a function on a website then returns with the data. This script works perfectly fine when I do not use a proxy by setting it to false. This is not a task that is NOT allowed to be done with Selenium/puppeteer

However, when I set a (working) proxy. It fails to perform the same task and is detected by the website firewall/antibot software.

Some things to note:
I have tried many (20+) different proxy providers (Residential and Datacenter) and they all have this issue
The issue does not occur if that proxy is set globally on my system
The issue does not occur if that proxy is set in a chrome extension
The SSL cipher suites do not match Chrome but they still don't match when not using a proxy so I assume that isn't the issue
It is very important to keep consistency in the header order
The question basically is. Does the request module change anything when using a proxy such as the header order?
Here is an image of what happens when it passes/fails.
The only difference is changing the proxy that causes this to fail. One request being made with, one request being made without.

After deactivating my old account I wanted to come back and give an actual answer to this question now I fully understand the answer. What I was asking one year ago was not possible, The antibot was fingerprinting me through the TLS ClientHello (And even slightly on the TCP/frame level).
To start, I wrote my a wrapper called request-curl which wrapped libcurl/curl binaries into a single library with the same format as request-promise, this gave me much more control over the request (preventing encoding, http2/proxy support and further session/TLS control) this still only let me reach a medicore rank of the 687th most popular ClientHello (https://client.tlsfingerprint.io:8443/). It wasn't good enough.
I had to move language. NodeJS is too much of a high-level language to allow for a really deep control (had to modify packets being sent from Layer 3). So as the answer to my question.
This is not yet possible to do in NodeJS - Let alone with the now unmaintained request.js library.
For anyone reading this, if you want to forge perfect requests to bypass antibot security you must move to a different language: I recommend utls in Golang or BouncyCastle in c#. Godspeed to you as it took me a year to really know how to do this. Even then, there's more internal issues these languages have and features they do not yet supposed (Go doesn't support 'basic' header-ordering, you need to monkey-patch/modify internals etc, utls doesn't easily support proxies). The list goes on and on.
If you're not already too deep into it, it's one hell of a rabbithole and I recommend you do not enter it.
According to the proxies documentation of the request module:
By default, when proxying http traffic, request will simply make a standard proxied http request. This is done by making the url section of the initial line of the request a fully qualified url to the endpoint.
Instead you can use a http tunnel by setting:
tunnel : true
in the request module proxy settings.
It could be that in your case, you are making a standard proxied http request, whereas when using a proxy globally on your system or a chrome extension a http tunnel is created.
From the documentation:
Note that, when using a tunneling proxy, the proxy-authorization header and any headers from custom proxyHeaderExclusiveList are never sent to the endpoint server, but only to the proxy server.
There are some scenarios that I can think of
Proxy is actually adding some headers to the final request (in order to identify you to the server)
The website you're trying to reach has your proxy IPs blacklisted (public/paid ones?)
It really depends on why you need to use that proxy
Is it because of network restrictions?
Is it because you want to hide the original request address?
Also, if you have control over the proxy server, can you log the requests being made to the final server?
My suggestion
Try writing your own proxy (a reverse one) and host it somewhere. Instead of requesting to https://target.com, to a request to your http[s]://proxy.com/ and let the reverse proxy do the work.
Also, remember to disable X headers on the implementation as it will change the request headers

You're using the http-scheme for you request, but if the webserver redirects http to https and if the proxy-server is not configured to accept redirects (to https) then the problem might only be about the scheme respectively the URL you enter.
So the proxy had to be configured to accept redirects or the URL has to be checked manually in the case of faults and then adjusted in the case of a redirect.
Here you can read about redirects on one proxy-server (Apache Traffic Server), the scenario there includes more redirects than I described above:

According to he page #Jannes Botis linked there exist still more proxy-settings that might be able to support or disrupt the desired functionality, so the whole issue is perhaps about configuring the proxy-server correct. Here are a few settings that are directly related to redirects:
followRedirect - follow HTTP 3xx responses as redirects (default: true). This property can also be implemented as function which gets response object as a single argument and should return true if redirects should continue or false otherwise.
followAllRedirects - follow non-GET HTTP 3xx responses as redirects (default: false)
followOriginalHttpMethod - by default we redirect to HTTP method GET. you can enable this property to redirect to the original HTTP method (default: false)
maxRedirects - the maximum number of redirects to follow (default: 10)
removeRefererHeader - removes the referer header when a redirect happens (default: false). Note: if true, referer header set in the initial request is preserved during redirect chain.
It's quite possible that other settings of the proxy-server have impact on fail or success of your scenario too.

mode: 'no-cors' won’t magically make things work. In fact it makes things worse, because one effect it has is to tell browsers, “Block my frontend JavaScript code from seeing contents of the response body and headers under all circumstances.” Of course you never want that.
What happens with cross-origin requests from frontend JavaScript is that browsers by default block frontend code from accessing resources cross-origin. If Access-Control-Allow-Origin is in a response, then browsers relax that blocking and allow your code to access the response.
But if a site sends no Access-Control-Allow-Origin in its responses, your frontend code can’t directly access responses from that site. In particular, you can’t fix it by specifying mode: 'no-cors' (in fact that’ll ensure your frontend code can’t access the response contents).
However, one thing that will work: if you send your request through a CORS proxy.
You can also easily deploy your own proxy to Heroku in just 2-3 minutes, with 5 commands:

Adding the proxy URL as a prefix causes the request to get made through your proxy, which:
Forwards the request to https://example.com.
Receives the response from https://example.com.
Adds the Access-Control-Allow-Origin header to the response.
Passes that response, with that added header, back to the requesting frontend code.
The browser then allows the frontend code to access the response, because that response with the Access-Control-Allow-Origin response header is what the browser sees.
This works even if the request is one that triggers browsers to do a CORS preflight OPTIONS request, because in that case, the proxy also sends back the Access-Control-Allow-Headers and Access-Control-Allow-Methods headers needed to make the preflight successful.

You don’t want to do that. To be clear, when you say you want to “disable CORS” it seems you actually mean you want to disable the same-origin policy. CORS itself is actually a way to do that — CORS is a way to loosen the same-origin policy, not a way to restrict it.
But anyway, it’s true you can—in your local environment—do suff like give a browser runtime flags to disable security and run insecurely, or you can install a browser extension locally to get around the same-origin policy, but all that does is change the situation just for you locally.
No matter what you change locally, anybody else trying to use your app is still going to run into the same-origin policy, and there’s no way you can disable that for other users of your app.
You most likely never want to use mode: 'no-cors' in practice except in a few limited cases, and even then only if you know exactly what you’re doing and what the effects are. That’s because what setting mode: 'no-cors' actually says to the browser is, “Block my frontend JavaScript code from looking into the contents of the response body and headers under all circumstances.” In most cases that’s obviously really not what you want.
As far as the cases when you would want to consider using mode: 'no-cors', see the answer at What limitations apply to opaque responses? for the details. The gist of it is:
In the limited case when you’re using JavaScript to put content from another origin into a <script>, <link rel=stylesheet>, <img>, <video>, <audio>, <object>, <embed>, or <iframe> element (which works because embedding of resources cross-origin is allowed for those)—but for some reason you don’t want to/can’t do that just by having the markup of the document use the resource URL as the href or src attribute for the element.
When the only thing you want to do with a resource is to cache it. As alluded to in What limitations apply to opaque responses?, in practice the scenario that’s for is when you’re using Service Workers, in which case the API that’s relevant is the Cache Storage API.
But even in those limited cases, there are some important gotchas to be aware of; see the answer at What limitations apply to opaque responses? for the details.

There is no 'opaque' request mode — opaque is instead just a property of the response, and browsers set that opaque property on responses from requests sent with no-cors mode.
But incidentally the word opaque is a pretty explicit signal about the nature of the response you end up with: “opaque” means you can’t see into any of its details; it blocks you from seeing.
If you are trying to address this issue temporarily on your localhost, you can use this chrome extension : Allow CORS Access-Control-Allow-Origin

If you are using Express as back-end you just have to install cors and import and use it in app.use(cors());.

So if you're like me and developing a website on localhost where you're trying to fetch data from Laravel API and use it in your Vue front-end, and you see this problem, here is how I solved it:
In your Laravel project, run command php artisan make:middleware Cors. This will create app/Http/Middleware/Cors.php for you.
Add the following code inside the handles function in Cors.php:

In app/Http/kernel.php, add the following entry in $routeMiddleware array:

(There would be other entries in the array like auth, guest etc. Also make sure you're doing this in app/Http/kernel.php because there is another kernel.php too in Laravel)
Add this middleware on route registration for all the routes where you want to allow access, like this:

Very easy solution (2 min to config) is to use local-ssl-proxy package from npm
The usage is straight pretty forward:
1. Install the package:
npm install -g local-ssl-proxy
2. While running your local-server mask it with the local-ssl-proxy --source 9001 --target 9000
P.S: Replace --target 9000 with the -- "number of your port" and --source 9001 with --source "number of your port +1"
Solution for me was to just do it server side
I used the C# WebClient library to get the data (in my case it was image data) and send it back to the client. There's probably something very similar in your chosen server-side language.

You can tweak it to whatever your own use case is. The main point is client.DownloadData() worked without any CORS errors. Typically CORS issues are only between websites, hence it being okay to make 'cross-site' requests from your server.

I had a similar problem with my browser debugger saying my response.body was null but fiddler and the developer tools show it as populated that turned out to be basically the same scenario as this. I was using a local Angular application hitting a Web Api service running on IISExpress. I fixed it by following the steps outlined here to find the correct applicationhost.config file to add a Access-Control-Allow-Origin header like so:

In my client code I am trying to use fetch to do the same thing, but I get the error:

So I am trying to pass in an object, to my Fetch which will disable CORS, like so:

Interestingly enough the error I get is actually a syntax error with this function. I am not sure my actual fetch is broken, because when I remove the { mode: 'no-cors' } object, and supply it with a different URL it works just fine.
I have also tried to pass in the object { mode: 'opaque'} , but this returns the original error from above.

mode: 'no-cors' won’t magically make things work. In fact it makes things worse, because one effect it has is to tell browsers, “Block my frontend JavaScript code from seeing contents of the response body and headers under all circumstances.” Of course you never want that.
What happens with cross-origin requests from frontend JavaScript is that browsers by default block frontend code from accessing resources cross-origin. If Access-Control-Allow-Origin is in a response, then browsers relax that blocking and allow your code to access the response.
But if a site sends no Access-Control-Allow-Origin in its responses, your frontend code can’t directly access responses from that site. In particular, you can’t fix it by specifying mode: 'no-cors' (in fact that’ll ensure your frontend code can’t access the response contents).
However, one thing that will work: if you send your request through a CORS proxy.
You can also easily deploy your own proxy to Heroku in just 2-3 minutes, with 5 commands:

After running those commands, you’ll end up with your own CORS Anywhere server running at, for example, https://cryptic-headland-94862.herokuapp.com/.
Prefix your request URL with your proxy URL; for example:
https://cryptic-headland-94862.herokuapp.com/https://example.com
Adding the proxy URL as a prefix causes the request to get made through your proxy, which:
Forwards the request to https://example.com.
Receives the response from https://example.com.
Adds the Access-Control-Allow-Origin header to the response.
Passes that response, with that added header, back to the requesting frontend code.
The browser then allows the frontend code to access the response, because that response with the Access-Control-Allow-Origin response header is what the browser sees.
This works even if the request is one that triggers browsers to do a CORS preflight OPTIONS request, because in that case, the proxy also sends back the Access-Control-Allow-Headers and Access-Control-Allow-Methods headers needed to make the preflight successful.

https://developer.mozilla.org/en-US/docs/Web/HTTP/Access_control_CORS explains why it is that even though you can access the response with Postman, browsers won’t let you access the response cross-origin from frontend JavaScript code running in a web app unless the response includes an Access-Control-Allow-Origin response header.
http://catfacts-api.appspot.com/api/facts?number=99 has no Access-Control-Allow-Origin response header, so there’s no way your frontend code can access the response cross-origin.
Your browser can get the response fine and you can see it in Postman and even in browser devtools—but that doesn’t mean browsers expose it to your code. They won’t, because it has no Access-Control-Allow-Origin response header. So you must instead use a proxy to get it.
The proxy makes the request to that site, gets the response, adds the Access-Control-Allow-Origin response header and any other CORS headers needed, then passes that back to your requesting code. And that response with the Access-Control-Allow-Origin header added is what the browser sees, so the browser lets your frontend code actually access the response.

You don’t want to do that. To be clear, when you say you want to “disable CORS” it seems you actually mean you want to disable the same-origin policy. CORS itself is actually a way to do that — CORS is a way to loosen the same-origin policy, not a way to restrict it.
But anyway, it’s true you can—in your local environment—do suff like give a browser runtime flags to disable security and run insecurely, or you can install a browser extension locally to get around the same-origin policy, but all that does is change the situation just for you locally.
No matter what you change locally, anybody else trying to use your app is still going to run into the same-origin policy, and there’s no way you can disable that for other users of your app.
You most likely never want to use mode: 'no-cors' in practice except in a few limited cases, and even then only if you know exactly what you’re doing and what the effects are. That’s because what setting mode: 'no-cors' actually says to the browser is, “Block my frontend JavaScript code from looking into the contents of the response body and headers under all circumstances.” In most cases that’s obviously really not what you want.
As far as the cases when you would want to consider using mode: 'no-cors', see the answer at What limitations apply to opaque responses? for the details. The gist of it is:
In the limited case when you’re using JavaScript to put content from another origin into a <script>, <link rel=stylesheet>, <img>, <video>, <audio>, <object>, <embed>, or <iframe> element (which works because embedding of resources cross-origin is allowed for those)—but for some reason you don’t want to/can’t do that just by having the markup of the document use the resource URL as the href or src attribute for the element.
When the only thing you want to do with a resource is to cache it. As alluded to in What limitations apply to opaque responses?, in practice the scenario that’s for is when you’re using Service Workers, in which case the API that’s relevant is the Cache Storage API.
But even in those limited cases, there are some important gotchas to be aware of; see the answer at What limitations apply to opaque responses? for the details.

There is no 'opaque' request mode — opaque is instead just a property of the response, and browsers set that opaque property on responses from requests sent with no-cors mode.
But incidentally the word opaque is a pretty explicit signal about the nature of the response you end up with: “opaque” means you can’t see into any of its details; it blocks you from seeing.

If you are using Express as back-end you just have to install cors and import and use it in app.use(cors());.

So if you're like me and developing a website on localhost where you're trying to fetch data from Laravel API and use it in your Vue front-end, and you see this problem, here is how I solved it:
In your Laravel project, run command php artisan make:middleware Cors. This will create app/Http/Middleware/Cors.php for you.
Add the following code inside the handles function in Cors.php:

In app/Http/kernel.php, add the following entry in $routeMiddleware array:

(There would be other entries in the array like auth, guest etc. Also make sure you're doing this in app/Http/kernel.php because there is another kernel.php too in Laravel)
Add this middleware on route registration for all the routes where you want to allow access, like this:

In Vue front-end, make sure you call this API in mounted() function and not in data(). Also make sure you use http:// or https:// with the URL in your fetch() call.

Very easy solution (2 min to config) is to use local-ssl-proxy package from npm
The usage is straight pretty forward:
1. Install the package:
npm install -g local-ssl-proxy
2. While running your local-server mask it with the local-ssl-proxy --source 9001 --target 9000
P.S: Replace --target 9000 with the -- "number of your port" and --source 9001 with --source "number of your port +1"
Solution for me was to just do it server side
I used the C# WebClient library to get the data (in my case it was image data) and send it back to the client. There's probably something very similar in your chosen server-side language.

You can tweak it to whatever your own use case is. The main point is client.DownloadData() worked without any CORS errors. Typically CORS issues are only between websites, hence it being okay to make 'cross-site' requests from your server.
Then the React fetch call is as simple as:

I had a similar problem with my browser debugger saying my response.body was null but fiddler and the developer tools show it as populated that turned out to be basically the same scenario as this. I was using a local Angular application hitting a Web Api service running on IISExpress. I fixed it by following the steps outlined here to find the correct applicationhost.config file to add a Access-Control-Allow-Origin header like so:

I'm trying to use jquery.couch.js to do couch operations in my ember.js app, but I'm having cors problems, and I have no clue what a good solution is.
It seems to me that couch running on port 5984 would make it basically unusable? Why do requests to different ports cause cors problems? And how on earth do OTHER people end up getting couch to work? I'm immensely confused, and not sure how to proceed.
My couch instance returns this from curl:

Help me! I humbly recognize my noobiness for saying this, but how is couchdb even useful if this is built right into the basic functionality?

I just now tried to add crossDomain: true, xhrFields: {withCredentials: true} to my login call, to no avail. Exact same error message. I'm clearly missing a core concept.
The message you are seeing is referring to the server, not the client. Changes made to the client's call will not, as you reported, change the result.
In CouchDB 1.4 specifically, CORS support must be explicitly enabled and an origins declaration must be made. That said, depending on how you are using your CouchDB instance there are two ways to enable it:
Change the setting in your local.ini directly and restart your instance, see here for more info: http://wiki.apache.org/couchdb/CORS
In the case you have futon available, go to Settings and find the setting there and enable it, in this case no restart is needed.
Update
It seems that the CORS section is not always existent by default, in this case just add it yourself.

For those who are using Cookie authentication (not password authentication) and are reusing the cookie in the Ajax request returned by the CouchDB server, you still need to do this in your $.ajax() requests to CouchDB:
xhrFields: {withCredentials: true},
Which, means you have to open the jquery.couch.js file that you sourced from the couch server and manually insert that option into the javascript.
CORS didn't work for me without both doing this on the client side and setting "credentials=true" on the server side.
The original jquery.couch.js as it is written right now doesn't support the client side sending Cookies with CORS, so you have to do it yourself until someone opens a ticket to get this fixed.
I'm hoping someone can answer this question for me, I'm not an expert on servers so please excuse me if I'm completely off base.
I'm using Android webview (PhoneGap 1.4.1) to make Ajax calls but I keep getting a ready state 4 status 0 on each call. I've spent the last couple hours investigating this and I may have figured out why. I used xhaus.com/headers to check my requests and found that in web view my "Accept" header is:
text/xml, text/html, application/xhtml+xml, image/png, text/plain, /;q=0.8
however, if I pull up the Android browser and check my header that way, I see that my "Accept" header is:
application/xml, application/xhtml+xml, text/html;q=0.9, text/plain;q=0.8, image/png, /;q=0.5
I checked the server that is providing the XML and found that the return heads are "Content-Type" is set to:
application/xml
My first question is: Webview doesn't seem to support "application/xml" type, so could this be the reason I'm having my issue? Or am I completely off base here?
Second question: Is there anything I can do on the client side to fix this or will the server admin need to make the change? I am using GET to make the request.
Third question: Is this normal? why would web view / browser have this sort of mismatch?
My app has been tested on 10+ handsets and only 2 have this issue... Very strange.

I may be wrong, but it sounds like you're calling a web service? Some services support mutliple calling strategies depending on the content-type header that you pass.
For example SharePoint web services support both SOAP 1.0 (if Content-type is sent as "text/xml; charset=utf-8") and SOAP 1.2 (if Content-type is set as "application/soap+xml").
In your case I would try setting a content-type of "text/xml; charset=utf-8" and see what happens.
As for the "accept" header if the server response isn't acceptable based on the passed value it should pass a status of 406 (not acceptable). It may simply be a bug in the server or in any of the steps between tho' that it's not. Using a tool like Fiddler you should be able to recreate both requests (both variations on "accept") and see exactly what the server responses with however - that may be the easiest way to truly get to the bottom of things.
