I face problem with reset value to empty with jquery when click radio button.

Make sure you are including JQuery in your page.
To bind your event, you need to wait that the DOM is fully loaded, else you would try to use an element that doesn't exist yet.

Also, you might want to change the JQuery selector from ID to class, so that you can use the same code for all similar checkboxes.
Put a .question-checkbox class on the inputs, and remove all onclicks.
You don't have to call check on every click. Once document loads, call it once.

You're trying to listen an element id that does not exist(neu_ma) - you haven't given your checkbox an id.

Add some specific class to inputs that need to be affected by the state of your checkbox.
I made an example HTML:

What this code adds to your code is to have a value for the radio buttons. This allows us to make a selection based on which radio button was selected. Secondly, the input fields are grouped together in a <div> to allow for easy hiding of the three input fields. EDIT: Actually it's a submit page, the user enters tags in the #tag1 and when he clicks on submit I want to send these tags to my controller, that's why I'm using the hidden input...

I don't understand why you would want to copy the value of one input field to another (albeit, hidden). But if that is what you want to do, try using the below code.
The function attached to the onblur event of the input field would set the value of the input field to the hidden field whenever it loses focus.
The window.onload = setValue will do the same on page load.
