When logging in as the user that created the app, the manage_pages and publish_stream permissions are asked for and granted. When logging in as another user, the manage_pages permission is not asked for and not granted however publish_stream is. The code is below:

You need to submit a request for the 'manage_pages' permission to be granted to your app. Only then can you ask for that permission from other users besides the admin/developer of your app. Check the link below for more info:

I log the response to the console and receive the name and id, but not email address. How can I get the email as well? Upon researching it, it should get it by default, am I wrong?

By default, fb allows only basic permissions. What you require is additional permission, hence you have to request it this way.

You need to use FB.login function on click on fb connect button and don't use FB.getLoginStatus

If you log your response variable via console.log(response) you'll see what's wrong:
You'll get an error object with this message:
"An active access token must be used to query information about the current user."
So if you want to get information about the current user you have to send also an Access Token. To get more information about this check the Facebook JS SDK page.
You need to make sure that the user has logged into Facebook and authorized your app, before you call FB.api('/me', ...).

What is triggering the FB.api call? If it's just code within a tag then it's going to run as soon as the browser gets to that point.
You could possibly store some sort of cookie value or something after the FB.api call then check it on page load, but that seems like more work than is probably needed.
Do you want him to post it only once, ever?
If so, you're going to need to create a "state". In order to do this, you could do it client sided (with cookies), or server sided (with a database).
Create a boolean variable named "posted", and store it in a cookie or in a database (since you're using javascript, it's probably easier to use a cookie).
